This situation was probably brought about by Sandra's (the goblin) colleague Manuel the Scrivner, who has a hobby of studying monster girls but lacks Sandra's immortality which is necessary for internal examinations. Sandra got bored and figured there were better ways to pass the time.
Manuel's notes: It seems that her digestive tract really does extend into the tail. A fascinating adaptation which could prevent struggling prey from damaging internal organs and also allows good viewing of the bulge.

Artist note; I am terrible at scaling so Sandra should probably be much smaller and Uri should probably be much bigger.
